Office Hours

Our office is open Monday through Thursday from 7:00 AM to 5:00 PM. We are closed on Fridays. Our schedulers are available during office hours to assist you with your pre-visit registration and scheduling needs. Please call our scheduling team at 404-255-5956 and dial extension 906 when prompted by the voice attendant.

Referral and Medical Records

As specialists in rheumatology, we work closely with your other healthcare providers to coordinate the most comprehensive treatment plan for your condition. Therefore, we require a referral from either your primary care physician (PCP) or another specialist. Additionally, we request that you contact your referring physician, PCP, and any other physicians or specialists to request a copy of your recent medical records. Please have these documents sent to us at fax number 404-255-3908.

New Patient Paperwork

Prior to your visit, please ensure that all new patient forms, referrals, and any requested medical records are completed and received by our office before your appointment to avoid rescheduling delays. On the day of your appointment, please bring the following items:

• A complete list of all your medications, including the strength and dosage of each one
• Your health insurance card(s)
• A photo ID

When you check in, we will collect your co-payment, deductible, or any out-of-pocket financial responsibilities that are due prior to seeing the physician.

Patient Portal

Our practice utilizes the Patient Portal for all communications. If you need to contact our staff, see posted results when they become available, or send us documents, please use the Patient Portal. Our patient representatives are available to assist you in setting up access or addressing any issues you may have with the Patient Portal. Please dial 404-255-5956 extension 905 when prompted by the voice attendant. If you reach out to us by telephone and we are with another patient, your call will go to our voicemail system where you can leave a message, and we will call you back as quickly as possible. We kindly ask that you do not leave multiple telephone messages.
