Fees
ARAPB physicians participate with Medicare and several other insurance plans. Participation varies as changes occur within the insurance industry. Please check with our receptionists to confirm if we participate with your insurance plan.
For your convenience, we will file your primary insurance claims if you provide us with the correct billing information. We ask you to pay the deductible, co-insurance, or co-payment at the time of service. We accept Visa, Mastercard, cash, and checks. ARAPB physician do not participate with any Medicaid insurance products.

- Aetna HMO requires a referral from PCP (We do not participate with Aetna Medicare Advantage PPO - all “ALPHA” identification characters. If you see one of our physicians it will be out of network )

NOTE: Only Dr.’s Cerejo, Greer, Quintero, Schweitz & Tokayer participate with AVMED. All others are out of Network.
BCBS
- All Florida Blue: Blue Options PPO Plans
- BlueChoice HMO “Open Access” does not require referral
- Florida Blue PPO Medicare Advantage
- Florida Blue Health Options Medicare Advantage (HMO Requires a Referral)
- BlueCross BlueShield Out of State Plans (Some may require a referral and some plans require you to notify the home plan)
- All BCBS Federal Plans
- WE DO NOT TAKE BLUE CARE OR ANY COMMERCIAL HMO PLANS
CIGNA
- Most plans, some may require referrals
- We do not take LOCAL PLUS or EPO CONNECT (these plans do not have out of network benefits)
