This section offers detailed information regarding our accepted insurance policies, allows our patients to ask billing questions and make secure on-line payments.

• Accepted Insurance List - This list is our current accepted Insurance programs. Should you not be able to find your current insurance among this list, please call your insurance carrier and provide them with our Tax Identification Number # 203926179.
  
• Billing Inquiries - After logging into your personal information, you will be able to make a secure payment on-line. The step by step instructions will be available once you have chosen this section.
• Disability Forms/ FMLA - Short term leave or FMLA as well as disability forms may be addressed with our physicians. The time frame for completion of these forms is 2 weeks.
• Please understand that all disability forms are not appropriate for our physicians to complete. Should our physicians not be able to be complete your requested form after reviewing, you will be notified. Please be aware there is a fee for our physicians to complete all Disability or FMLA forms, and must be paid in advance.

• Making A Payment - Log-in to your secure personal information and select to make your on-line payment. Step by Step instructions are provided in this section. 
• Medical Records Release Forms - Health Records are protected by Federal Regulations, therefore we are not able to forward your Health Record Information to any third party without your consent. The only exception to this regulation is if we receive a subpoena requesting your file.
